2027 PLAYER REGISTRATION & WAIVER FORM

- Required for all new & returning players.
- Completed form and payment must be received by March 1st, 2027 to be included in the draft and not incur a $10.00 late fee.
Drafts will be 1st week in April, 2027. -
- Payment can be made by check or cash.
- Please write what Payment App you paid with and the date paid if not mailing a check or cash with your form.
